The roller coaster ride that is RA Options

Hi everyone,
I've not been too good of late as my joints are flaring on and off. My Rheumatology Nurse believes that the Rituximab is no longer working for me which is a familiar pattern that I seem to follow with all the disease modifying drugs/biologics etc. I start off on them with great results which lifts me up only to have them fail me in some way later on which brings me crashing back down Sad I am so fed up of this roller coaster ride as each flare leaves me feeling more and more exhausted and depressed. I hate how RA messes with my head! One minute I'm feeling great and ready to take on the world and the next I'm deep in the darkest depths of despair. Does RA leave anyone else feeling like this or is it just me feeling sorry for myself?

H Wendy, I am sorry to hear, you are having such a rough time. Unfortunately it sounds so very familiar. I am in exactly in the same situation.

I have been on the anti-tnf Cimzia injection for 2 years, and it has been up and down all the way, reflected in my CRP blood test. It can be down to 4 one month and then rise to 78 the next. At the moment it is 28, and I feel awful: pain in my shoulders, my knuckles, have difficulty walking, because of my knees. Had one knee drained 5 weeks ago with a steroid injection, but it did not help much. On top of that the awful tiredness, I am sleeping 10-12 hours a night, when my shoulder joint lets me.

The doctor is now talking about Rituximab, which you say, you have tried. How long have you been on it? I have not been so keen, as it seems so involved.

I am hoping, when the warmer weather finally arrives, we will all start to feel better. this cold and gloomy weather really gets me down. We need the warm sun with D-vitamin.

Hi Merete,
I'm sorry too that you are having a rough time too. I agree about the weather and needing some sunshine, roll on the summer...
I have been on Rituximab since January 2012 and my first infusions were a success. Side effects were minimal (headache/tiredness after infusions) However, the last infusions I had in October 2012 have not had the desired effect. I have also had quite a few infections since November, mainly upper respiratory. I don't see my consultant until the 22nd of this month so my nurse suggested a two week course of oral steroids to tide me over which I stop taking after this weekend. They are suggesting maybe another shot at the Rituximab or changing to Tocilizumab. Right now, I would try anything that has the potential to make me feel better. I hope too that you find something that suits you Merete.

I feel the ups and downs but I've just had a really helpful programme run at the hospital.
It's worked on my mental capacity to cope with the wild swings of thinking I'm super woman that can and will conquer all to being a rediculous feeling heap

Basically it's a CBT programme run by the pain clinic
I needed some help to even things out and do you know, it's really Been a big help and the techniques to tackle unhelpful thinking and recognising the cycles of thinking, pain and behaviour is part of this process.

Try and get referred to the pain clinic

You may already be doing this, but I have heard that a combination of DMARDs and biologics can be better than the sum of its parts.
